  • Approved and currently registered with South Carolina Secretary of State's Office as Non-Profit/501(c)(3)

Group Information

-PLEASE NOTE-

  •  Internal D7 group (i.e. Orchestra, Band, Teacher Cadets, etc.), please provide the Group name and other required information.
  • Individual D7 staff member who will be submitting requests on your own behalf (i.e. not for a group, team, club, etc), use your first and last name as the Group Name. 
  • All outside/non-D7 organizations, provide the Group name and other required information.

Additional Group Information

External / Outside Groups: Upload the Group's Certificate of Insurance and its Expiration Date.

-PLEASE NOTE-
CERTIFICATE OF LIABILITY INSURANCE
The district will review your group application; however, for all external, non-D7 groups. the district cannot approve any request to use a facility until you submit a valid Certificate of Liability Insurance (above).

PROOF OF NON-PROFIT STATUS
Also, if you are registering a group as a non-profit, the district requires you to submit valid proof of the group's Non-Profit/501(c)(3) status (below).
